Children/young adults
Your doctor would like to know how you are doing and has therefore asked you to complete questionnaires on the KLIK website. You do this before your appointment in the hospital. Your doctor can view your answers on the questions and discuss this with you during the consultation. By this, the doctor knows how you are doing and can offer you the right help. Below you can find a video in which KLIK is explained.

Using KLIK without an account
It is possible to complete questionnaires without a KLIK account. The answers on the questionnaires are immediately fed back and can be printed and brought to the consultation. In this way, you (or your parents) can see how you are doing and you can discuss the topics from the questionnaires with your doctor. The questionnaire that is now available is the Pediatric Quality of Life Inventory (PedsQL), a questionnaire that measures quality of life of children that are (or have been) under treatment in a hospital. In addition, it is possible to write down a question for the doctor, so you won’t forget to ask this question during the consultation. Below you can find the link of the questionnaire, so you can try it right away!
Well prepared to the consultation
Sometimes not everything you find important is discussed during the consultation with your doctor. In the animation video below tips and tricks are provided about how you can prepare yourself for the consultation with your doctor, so everything you find important is discussed. Theme list
The theme list that is mentioned in the video can be found here. You can print this list at home and indicate what you would like to discuss during the appointment in the hospital. You can also write down some questions on this list. Take the printed theme list with you to the appointment with your doctor.
